Most common cause is the server is just overloaded. That can happen for lots of reasons including too many people trying to access, server undersized to begin with, other scheduled workloads going on (like a backup running) which increases load. Fix can be approached in many ways including just moving to a bigger server, multiple servers with an up front load balancer, switching architecturally to a more fault tolerant configuration.
Basically, the system can be made as fast and as reliable as you can afford. Always a balancing act between maintaining a reasonable cost for the system and performance/reliability of the system.
