SBR Form 1 and installation... Where to go?

I need to get a 10.5" barrel installed on my AR and have the appropriate SBR form(s) filled out. I have heard of a place that does both for a reasonable price. Any suggestions?

Thanks to everyone that PM'ed and posted in this thread. I have succesfully set up my NFA Firearms Trust with Bank America. I should have my checks this week and then I will submit my SBR form for approval. I keep you all posted onthe profgess on the rifle build as it comes along.
