I am thinking the bullet didn't completely go into the chamber. Either to long or had a buldge in the case but it had to be close to closed as it cant fire it the bolt isnt almost closed.
Nope we were all shooting 9's, maybe one shot a .40 for a minute.Somebody drop a 9mm round in a .45?
I was shooting a steel target and the shot prior was a hit, then the out of battery. Once I cleared the gun the first thing I did was check the barrel to make sure it wasn't blocked.Could the round shot previously have been a squib?
I did clean it after this happened. The firing pin wasn't very dirty or damaged. I'm really at a lossPossibility of the firing pin is/was stuck in the forward position?? You may have had a slam fire, which has out of battery detonation tendency. Look at the bolt head and see if firing pin is slightly sticking proud of the bolt face. Strip the bolt down, give a good cleaning and lube as required.
