I was going through some reloading gear a friend had given me recently (yes, someone is actually getting OUT of reloading) and came across some loaded rounds of 223 from years past. I asked about them and he said they were from when he changed presses and forgot to reset his dies...

Note the top round with the bulged case near the almost absent shoulders and the crimp on the bullet. It also looks like the neck may have been too long from the case not being trimmed before loading. The bottom round was properly loaded. This is why I reset all my dies when I move them (I also don't use powder and primers when I am setting up my dies).
