With all due respect to THREEJAYS , I think his fancy shamcy computer may have posted
the photo in a misaligned way.
I do believe it is an old fashioned crank & yanker for the flattened nut*.
It comes in 2 different verisons (1 for the left nut and 1 for the right nut).
It can also be used as a nose hair puller, but only by a certified mechanic/nose twister
as seen in the photo below of just such a world wide known person...
