I wonder if he was watching tv while priming and managed to push a primer into an already primed case. Even still, after looking at my hand primer, the extension cuts off access to the tray once you squeeze the lever. Freak accidents happen.

When I was living in an apartment I did both, putting a thick rubber mat cut down on a majority of noise vs placing the tumblers on a carpet/tile/concrete floor.
Someone with more precision shooting experience will jump in. But did you look at the OAL of the factory loaded ammo? Breaking out the chrono will help you see how you loads are acting against the factory ammo.
I'm going to start making my own soon as well, but until then I've had great luck with Carolina Brass.

Are you talking about the lee loader? Its the one where you use a rubber mallet to accomplish much of the loading.
Or the Hand Press? Basically a single stage press that is handheld.
I have experience with both, and after taking a 4 year hiatus from reloading have gone back to the hand...
