22 bullets tumbling-AGAIN!!

I need your help! Bought a 22 can, put it on a Ruger sr-22, ruger 10/22 and a cz 455. Bullets were key-holing at 5 yards. Using cci standard velocity, cci sub-sonic and some federal bulk ammo. Contacted manufacturer, shipped it back. Got a entirely new can (obviously same stamp #s) went and shot again today and bullets are key-holing AGAIN! WTF!! I am at a loss. I do NOT have any baffle strikes. Pull the can off and bullets quit tumbling. Please god somebody tell me that i have over looked something simple so i can just take my kids out and shoot steel and enjoy it.

I have scoured these baffles and I dont see any contact anywhere. All of the hosts are factory threaded.

I cant tell if its lead or whatever material the end cap is made of. Makes absolutely no sense why it would strike the cap and not touch any of the baffles unless the cap is drilled just slightly off centered.
