Continue driving until extreme heat drops the seat out of the head.
No clue as to how this happened.
My only speculation is the engine overheard the driver saying how he's ok with some gun control laws for safety of children and greater good...

That's reverse lockout, to prevent accidental 5th to reverse shift.
Which is silly worry cause for max. acceleration, you'd go 5th to 3rd, red line the engine in 3rd, THEN shift into 4th. ☺
Bit of maintenance will go long ways...
There's no engine that'll stay together when repeatedly pushed past it's limitations.
Had to go dig this one back out for pic. 3UR/ 5.7L with 6.0L stroker kit installed for forced induction. Machine shop work and assembly was done by Mickey mouse. ARP...

Your 2UZ is almost as clean as 3RZ/ 2.7L in my 1998 Tacoma with 270k.

Both are tough engines. Please take your time and find a well maintained one, unless you're looking at a new one. I know you treat your vehicle(s) well.

Someone has out done @ItBeMe. With unwavering commitment and dedication, simply refused to change oil in his/ hers 2016 3UR-FE/ 5.7L to the point oil could not drain out of the crankcase...
Most repairable engine failures don't come to me. Ones that suffered catastrophic failures do.
Don't get to see too many normal overheated engines, often melted and seized.
This one was bit painful, not the usual neglect and/ or abuse. Or end of service life.

16V / 4 Cyl, the head is off of 2AZ-FE. I believe it had slow leak at the water pump, owner kept adding water into radiator lowering the boiling temperature. Metal head gasket held on as steam erode into #2 and #3 cyl.
Should have called me, I got bunch of fan brackets. ( better known as fake water pump )
Water pump is behind the fan bracket driven by the timing belt.
This is 3MZ-FE but 5VZ-FE is near identical in configuration.
